2020-2021 D'Youville Student-Athlete Medical Information

D'Youville College student-athletes are required to provide annual medical physicals and insurance information that includes coverage for athletically-related injuries. This is a pre-requisite for practice and competition. No student will be allowed to participate, in any way, until such evidence of current insurance coverage and medical information is on file with the D'Youville College Department of Athletics.


Student-Athletes can fill out paperwork through the new ATS athlete portal. The attached documents below will give you detailed instructions on how to complete medical forms to be eligible to participate in athletics at D’Youville for both new and returning student-athletes.

Along with medical forms ALL student-athletes must have an up-to-date yearly physical, please use the attached physical form we provide. We ask that you use our physical form because you must have a SPORTS PHYSICAL that states you are cleared to participate in intercollegiate athletics. ALL OTHER PHYSICALS WILL NOT BE ACCEPTED! You must send a hard copy of the physical to Kevin Pullyblank, Head Athletic Trainer by fax, mail, or email.


D’Youville Sports Physical Form


If you cannot receive your yearly sport physical until after the due date for medical forms because of insurance reasons, please do one of the following:

  • If you are a returning student-athlete remind the athletic training staff that you have an up-to-date physical on file from last year. Remember to update your physical by the expiration date of your existing physical.
  • If you are a new student-athlete please provide us with a current physical copy that you have had within one year and make plans for providing us with up-to-date physical by the expiration date of your existing physical.

*Student-athlete sport physicals are to be sent to the athletic training department NOT THE HEALTH CENTER. The health center has their own requirements that differ from the requirements we have for student-athletes.*


NCAA Catastrophic Injury Insurance Program

The NCAA's Catastrophic Injury Insurance Program covers student-athletes who are catastrophically injured while participating in a covered intercollegiate athletic activity (subject to all policy terms and conditions). This coverage does not qualify as the basic coverage required for participation in athletics at D'Youville College. Please complete all medical forms on the ATS athlete portal and get sports physicals in by September 11, 2020. Please send documents to the D'Youville Athletics Athletic Training staff at the mailing address or fax number listed below.
